Transitions of Patient Care: A Standardized Tool for Provider Handoff

      Highlights

      • Lack of communication during patient care transitions has a negative impact on provider satisfaction and patient outcomes.
      • The use of standardized verbal and written handoff procedures improved advanced practice provider satisfaction for neurosurgery patients.
      • Standardized handoff processes can be used in many patient populations to improve provider satisfaction. Additional studies are needed to determine whether this standardization can improve patient outcomes.

      Abstract

      Lack of communication during care transitions leads to provider dissatisfaction and adverse outcomes. The neurosurgical advanced practice providers (APPs) at this institution reported low satisfaction with handoff between units. The objective of this quality improvement initiative was to develop a standardized handoff process to improve transitions of patient care and increase neurosurgical APP satisfaction. The process included verbal handoff from a neurocritical care APP to a neurosurgery APP and a written transfer summary. High adherence to the initiative was achieved, with improved provider satisfaction post-implementation. This quality improvement initiative demonstrated improved APP satisfaction using a standardized handoff process.
